The article examines the theoretical and applied foundations of the institutional development of the administrative mechanism of public healthcare governance under the conditions of digital transformation, decentralization, and Ukraine’s post-war recovery. The current state of the healthcare system’s institutional framework is analyzed, key trends in its development are identified, and the factors influencing the effectiveness of public policy implementation in the healthcare sector are systematized. Particular attention is paid to assessing the role of multi-level governance, digitalization of administrative processes, strategic purchasing of healthcare services, and the institutional capacity of local communities in modernizing public healthcare governance. A comprehensive conceptual approach to improving the administrative mechanism of public healthcare governance is proposed, integrating institutional, organizational, digital, and managerial components into a unified development model. Unlike existing approaches, the proposed concept combines the principles of institutional resilience, intersectoral cooperation, digital integration, and multi-level governance as complementary elements of an effective public administration system in the healthcare sector. The practical value of the research lies in the possibility of applying the proposed conceptual provisions and strategic directions in the development of healthcare policies, modernization of public governance mechanisms, improvement of the performance of public authorities, and further alignment of Ukraine’s healthcare governance system with European standards of public administration.
